Every hip replacement — regardless of surgical approach — involves some degree of soft-tissue disturbance, and the body's response to that disturbance is inflammation. This is a normal healing mechanism, not a sign that something has gone wrong.

When the surgical site is disturbed, the immune system triggers an acute inflammatory cascade: blood vessels in the area become more permeable, allowing fluid and white blood cells to move into the surrounding tissue. The result is localised swelling around the hip and thigh. Because the hip connects directly to the entire lower limb, this fluid often tracks further down — into the knee, and sometimes as far as the calf, ankle, or foot.

Practical steps to help reduce swelling at home

Movement is the single most effective tool available during home recovery — more so than any passive measure. Gentle, progressive walking, built up gradually as the physiotherapy team advises, activates the lower-limb muscle pump that drains accumulated fluid from the thigh and calf. Each short walk does quiet circulatory work that rest alone cannot replicate.

That said, rest matters too — especially in the first couple of weeks. When sitting or lying down, raising the operated leg so that the foot is above heart level helps gravity assist fluid drainage away from the ankle and knee. None of these measures is complicated. Consistently applied alongside structured physiotherapy, they give the body the best conditions to resolve swelling at its own pace.

How the surgical approach may influence swelling

The type of hip replacement performed affects how much soft-tissue disruption the body has to manage — and that, in turn, shapes the scale of the early inflammatory response.

In a traditional posterior approach, the short external rotator tendons at the back of the hip — including the piriformis — are divided to access the joint. Those severed tendons and the posterior capsule then require roughly 90 days to fully heal, fibrose, and reattach. That tissue severance adds to the body's initial inflammatory burden, contributing to greater early swelling and a longer period before those structures are functionally restored.

Because the hip's proprioceptive structures remain intact after SPAIRE, patients are typically able to mobilise earlier and with greater confidence. Earlier walking activates the lower-limb venous and lymphatic pump — the same mechanism that helps clear accumulated fluid from the thigh and calf.

That kind of specific observation gives the clinical team something concrete to work with rather than a vague concern.
