What pain and swelling are normal in the first weeks

Swelling, bruising, and a persistent ache in the operated leg are to be expected in the weeks after hip replacement — they are a normal part of surgical healing, not a signal that something has gone wrong.

In the first fortnight, swelling is typically at its most noticeable. The operated leg and foot may feel puffy and heavy, and some patients notice warmth around the hip and upper thigh. Bruising is common and can migrate downward toward the knee in the days after surgery — this happens because blood from the surgical dissection tracks through tissue planes and is not itself a complication. The discolouration tends to fade over one to two weeks as the body reabsorbs it.

Beyond the first two weeks, swelling usually begins to settle gradually — but gradually is the operative word. It rarely disappears overnight and, in some patients, particularly those with lower baseline fitness or greater pre-existing inflammation, mild puffiness can persist for six to twelve weeks or longer. An ongoing ache during this period, especially after a longer walk or a physiotherapy session, is also typical. Activity stirs up inflammation in healing tissue, so feeling more sore after increased exertion is a pattern most patients experience rather than a warning sign.

Mild stiffness on rising from a chair or after rest is similarly common in the first month and reflects the natural rhythm of early recovery rather than any structural problem.

How long swelling and discomfort typically last

Thinking of recovery as a series of phases can help manage expectations without unnecessary anxiety.

Weeks 1–2 are typically the most demanding. Swelling is at its peak, the leg may feel heavy and uncomfortable, and fatigue is common. Short indoor walks and prescribed exercises are the priority during this phase — building confidence gradually rather than pushing through significant discomfort.

Weeks 3–6 generally bring visible improvement. Swelling begins to ease, pain becomes more manageable with lighter analgesia, and most patients return to light daily activities and short outdoor walks. Many find they are driving again toward the end of this window, once clear of opioid medication and able to perform an emergency stop — though the surgical team should confirm the appropriate time at follow-up.

Within that six-to-twelve-week window established as the typical resolution range, swelling has settled for the majority of patients. Some, however — particularly those with lower pre-operative fitness or greater baseline inflammation — may notice mild puffiness persisting beyond three months. This is not automatically a cause for concern, though any sudden or worsening swelling warrants prompt clinical review.

Throughout all phases, swelling tends to be more noticeable by the end of the day or after extended time on your feet. That pattern is common and expected. The reliable signal to watch is steady overall improvement week on week, even when individual days vary.

Managing pain at home to support recovery

The analgesic plan you leave hospital with is not simply there to make rest more comfortable — it is designed to keep you moving. Pain that is poorly controlled discourages walking, delays physiotherapy, and slows the muscle rebuilding that underpins the whole recovery. Taking analgesia consistently, rather than waiting until discomfort becomes severe, is how most patients stay able to engage with their exercises and build confidence on their feet in the early weeks.

Beyond medication, a few practical measures can make a meaningful difference to day-to-day discomfort. Elevating the operated leg when resting — ideally with the foot raised above hip level — helps fluid drain away from the ankle and foot and can reduce end-of-day heaviness. Pacing activity, rather than completing all tasks at once, avoids the flare of swelling and soreness that tends to follow a long, unbroken spell on your feet. Short walks followed by a rest period are more productive than one prolonged effort.

Rest is not a passive part of recovery. In the early weeks, the tissue around the hip is actively remodelling, and recovery happens during the periods between movement — not only during exercise itself.

Warning signs: when swelling or pain needs urgent attention

Steady week-on-week improvement is the expected pattern — but certain symptoms should never be watched and waited on at home.

Deep vein thrombosis (DVT)

DVT is a known risk after hip surgery. Unlike the diffuse puffiness of normal post-operative swelling, DVT typically presents in one leg only and tends to worsen rather than fluctuate. The signs to act on immediately are:

  • New or worsening severe pain in the hip, thigh, or calf
  • Swelling that is asymmetric — one leg significantly more swollen than the other
  • Redness, warmth, or visibly enlarged veins in the affected leg
  • Calf tenderness when you flex your foot

If a clot dislodges and travels to the lungs, it becomes a pulmonary embolism — a medical emergency. Do not wait to see if these symptoms ease. Contact your surgical team or NHS 111 the same day.

Wound and infection signs

Some warmth and minor redness around a healing incision is common in the first week. What requires prompt reporting is a change — particularly:

  • Increasing redness spreading beyond the wound edges
  • Discharge from the wound site
  • Fever or chills
  • Heat that is localised and worsening

These may indicate a surgical site infection. Contact your surgical team directly; do not monitor at home.

Emergency symptoms — call 999

Sudden severe deterioration, chest pain, or breathlessness requires emergency services immediately. Breathlessness after hip surgery can be a sign of pulmonary embolism. Sudden collapse or rapidly spreading swelling across both legs also warrants a 999 call rather than a phone consultation.

The rule is straightforward: for new or worsening pain, calf symptoms, fever, or wound changes — call your surgical team or NHS 111. For breathlessness, chest pain, or sudden collapse — call 999 or go directly to A&E.

How a muscle-sparing approach may influence early pain and swelling

Understanding what drives early pain and swelling also helps explain why recovery does not look the same for every patient — and why the surgical approach your team used matters beyond the operating theatre.

In a conventional posterior hip replacement, the piriformis and obturator internus tendons — key stabilisers at the back of the hip — are divided to access the joint. Severing them creates an additional layer of surgical trauma beyond the joint replacement itself, and the divided tissues then require roughly 90 days to heal. During that window, traditional protocols typically impose strict movement restrictions: no hip flexion beyond 90 degrees, no crossing the legs. Those precautions exist because the hip's own soft-tissue stability has been temporarily disrupted.

The SPAIRE technique, developed and published by Professor Paul Lee, addresses this differently. By preserving the piriformis and obturator internus while repairing the obturator externus, the procedure avoids that layer of tendon division. Less tissue divided means less localised inflammation from surgical trauma — and, for suitable patients within this pathway, often no requirement for the restrictive precautions that conventional posterior surgery imposes. Preserved tendons also maintain the hip's positional sense: the joint can instinctively recognise and protect itself against destabilising movements, which reduces the anxiety that commonly stalls early progress.

Professor Lee's broader recovery-by-design framework treats technique as one component alongside implant choice, mobilisation planning, and structured physiotherapy — not a stand-alone promise.

If you are still deciding on surgery, asking your surgeon specifically about the access approach they plan to use — and what it means for early movement and any precautions — is a direct and worthwhile question. If you have already had surgery via this pathway, the absence of mandatory hip precautions is an intended feature of the design: move with the confidence your clinical team has guided you towards.

Questions worth raising at your follow-up appointment

Your post-operative review is more useful if you arrive with something to report — not just a vague sense of how things are going.

In the days before the appointment, note down:

  • Whether swelling and pain have been following an improving trend, or whether progress has stalled or reversed at any point
  • Specific activities that provoke discomfort — stairs, sitting for long periods, getting in and out of a car
  • Any concerns about the wound site, including persistent redness or unusual sensitivity
  • How far and how confidently you are walking, and whether slopes or uneven ground feel manageable

The follow-up is also the right moment to ask about returning to driving, desk work, or any travel you have planned. These questions are expected and will be answered in the context of your individual progress — your surgical team is not looking for a minimum recovery standard but for evidence that your recovery is on track.

Physiotherapy progress is worth raising directly: if certain exercises feel impossible or exercises that once helped have stopped working, say so.

One reminder worth repeating: if symptoms worsen before the scheduled appointment — new calf pain, spreading redness, or fever — do not wait for that date. Contact your surgical team or NHS 111 directly.
